DVA Home Care Melbourne — Registered Veterans' Care Provider

Luxe Care is a registered DVA Community Nursing provider, supporting veterans and their families across Melbourne and the Mornington Peninsula. For eligible Gold and White Card holders, DVA-funded community nursing provides clinical care and personal support at home with no out-of-pocket cost.

Every Luxe Care care manager is a registered nurse or brings extensive nursing experience to the role. Your care is coordinated by people who understand both the clinical realities of service-related and age-related conditions, and the practical details of living well at home.

What Is the DVA Community Nursing Program?

The DVA Community Nursing Program is funded by the Department of Veterans Affairs and provides clinical nursing and personal care services to eligible veterans in their own home.

 

It is designed for veterans who have assessed clinical needs, and it covers a meaningful range of support from wound care and medication management through to personal care and recovery nursing after a hospital stay.

 

The Program is entirely free for eligible Gold Card and White Card holders. DVA pays the provider directly. There is no gap fee, no co-payment, and no means test.

IMPORTANT UPDATES TO VETERANS'
HOME CARE SERVICES

Recent changes to the veterans entitlement system, including the move to the Military Rehabilitation and Compensation Act from 1 July 2026, do not affect the DVA Community Nursing Program.
 

The Program continues exactly as before, and nothing changes for veterans currently receiving community nursing. If you are already a Luxe Care client, your care continues without interruption. Who Is Eligible for DVA
Community Nursing?

​DVA Community Nursing is available to veterans who hold a current Veteran Card and have an assessed clinical need for the service.

  • Veteran Gold Card (also known as the Veteran Card, All Conditions): covers all medical conditions, including those not related to service. The most common pathway for Gold Card holders to access community nursing.

  • Veteran White Card (also known as the Veteran Card, Specific Conditions): DVA funds community nursing where the care needed is due to an accepted service-related injury or condition. Luxe Care will confirm eligibility with DVA before commencing care for White Card holders.

  • War widows and widowers: eligible war widows and widowers may access continued DVA-funded support. Speak with Luxe Care or contact DVA on 1800 838 372 to confirm your specific entitlements.

From 1 July 2026, veterans with conditions accepted under the Veterans Entitlements Act who were previously assessed under the DRCA may now be eligible to apply for higher levels of support under the MRCA framework. What Does DVA Community
Nursing Cover?

The DVA Community Nursing Program covers clinical and personal care services delivered in your home by qualified nurses. Care is tailored to your assessed needs and reviewed regularly to make sure it continues to match your situation.

  • Clinical nursing
    Wound care and dressing changes, post-surgical recovery nursing, catheter and continence care, IV therapy and medication management, chronic disease monitoring, palliative care nursing

  • Personal care
    Assistance with showering, bathing, and personal hygiene, dressing and grooming, mobility transfers and moving safely around the home

  • Post-hospital recovery
    Nursing visits following discharge from hospital or day surgery, coordination with your treating team, monitoring for signs of complication or deterioration

  • Chronic and complex conditions 
    Ongoing nursing support for veterans managing complex conditions including acquired disability, neurological conditions, and service-related injuries requiring regular clinical attention

  • Care coordination 
    Care plan development, liaison with your GP and treating specialists, written updates to your medical team, coordination of allied health services where needed.

How to Access DVA Community Nursing Through Luxe Care

Accessing DVA-funded community nursing involves a referral from your GP or treating medical practitioner. Luxe Care handles the paperwork and coordinates with DVA directly, so you do not need to navigate the process alone.

  1. Confirm your card. You will need a current Veteran Gold Card or Veteran White Card.

  2. Get a referral. Ask your GP, hospital discharge planner, nurse practitioner, or treating doctor to complete a written referral for DVA Community Nursing services. Referrals can be made using the DVA D9389 Community Nursing Referral form or on the referring practitioner's own letterhead.

  3. Nominate Luxe Care. Ask your referring practitioner to send the referral directly to Luxe Care, or call us on 1300 674 886 and we will guide your GP through the process.

  4. Clinical assessment. A Luxe Care registered nurse will visit you at home to assess your clinical needs and build your care plan.

  5. Care begins. Once your care plan is in place and DVA approval is confirmed, your regular nursing and personal care visits commence. Your GP retains clinical oversight throughout.

 

Referrals are valid for 12 months and are renewed annually. Luxe Care manages the renewal process with your GP so there is no interruption to your care.

VETERANS HOME CARE SERVICE FAQS

  • Yes. The DVA Community Nursing Program funds clinical nursing and personal care services at home for eligible Gold Card and White Card holders. Care is provided by qualified nurses and is entirely free for eligible veterans. There is no co-payment and no means test.

  • Yes. A written referral from an authorised referrer is required for DVA Community Nursing. Authorised referrers include your GP, a treating doctor in hospital, a hospital discharge planner, or a nurse practitioner specialising in community nursing. Luxe Care can help coordinate this process with your GP.

  • No. The DVA Community Nursing Program is unaffected by the legislative changes that commenced on 1 July 2026. Your community nursing care continues exactly as before. However, if you are a veteran with conditions previously accepted only under the Veterans Entitlements Act, you may now be eligible for expanded support under the MRCA framework.   • Yes. Many Luxe Care clients receive DVA-funded community nursing alongside privately arranged services such as domestic assistance, companionship, or overnight support. These are coordinated under a single care plan managed by your Luxe Care Care Manager.

  • Once Luxe Care receives a valid written referral and DVA eligibility is confirmed, a clinical assessment is arranged and care typically commences within one to two weeks. For veterans with urgent needs, speak with our Care Managers on 1300 674 886 and we will work to prioritise your assessment.

  • Call 1300 674 886 and speak with a Care Manager. We will confirm your eligibility, explain what DVA Community Nursing covers in your situation, and coordinate the referral process with your GP. There is no obligation and no paperwork required from you to have an initial conversation.
