New Government End OF Life Funding
(Support at Home End of Life Pathway)

From 1 November 2025, older Australians can access a new Commonwealth Support at Home End-of-Life Pathway, which provides a one-off budget of $25,000 for additional help to stay at home at the end of life. This funding is designed to sit alongside existing state‑ funded‑ palliative care and hospital services, not replace them.


How the $25,000 funding works

  • Up to $25,000 is available per eligible person over a 12-week period, with flexibility to continue up to 16 weeks if funds are not fully used.

  • The budget can be used for Support At Home services such as nursing, personal care, domestic assistance, respite, transport and other practical help approved under the national Support at Home program.

  • The funding is time-limited and does not roll over once the end-of-life episode finishes.
     

You do not need to already be a Support at Home (or Home Care Package) client

Access to the End of‑ Life‑ Pathway does not require you to already have a Home Care Package (HCP) or Support at Home budget in place. People who are new to aged care at the end of life can be assessed and placed directly onto the End of‑ ‑Life Pathway so they can receive services quickly.


If someone is already receiving home care under Support at Home, their existing budget can be switched to the End of‑ Life‑ Pathway following a review so they can access the higher $25,000 allocation.

How to Fund In-Home Palliative Cares in Melbourne

Funding in-home palliative cares is more accessible than many families realise. Depending on your situation, there are three main pathways: government-subsidised Support at Home funding, Medicare rebates, and private care. Below is a plain-English

Support at Home End-of-Life Pathway — Up to $25,000

From 1 November 2025, the Australian Government's Support at Home program introduced a dedicated End-of-Life Pathway — the most significant aged care funding reform for palliative cares in a generation. It provides up to $25,000 in government-subsidised funding over a 12-week period, with flexibility to use the budget

up to 16 weeks if funds remain available.

 

Who is eligible?

You must be aged 65 or older (or 50 or older if you are Aboriginal or Torres Strait Islander), have been assessed by a doctor or nurse practitioner as likely to have 3 months or less to live, and have an Australian-modified Karnofsky Performance Status (AKPS) score of 40 or below — meaning you are in bed more than 50% of the time.

Do I need an existing Home Care Package to access it?

No. This is one of the most important points: people who are new to aged care can be assessed and placed directly onto the End-of-Life Pathway as a high priority, without first needing a Home Care Package or Support at Home classification in place. If you are already receiving Support at Home, your provider can request an urgent Support Plan Review to switch you across to this pathway.

 

What does the $25,000 cover?

The budget can be used for Support at Home services including: nursing care, personal care, domestic assistance, respite care, transport, and other approved practical support. Specialist palliative care such as symptom management and medications is funded separately by state and territory programs and runs alongside — not from — this $25,000 budget.

 

How to access it?

Your GP or nurse practitioner completes an End-of-Life Pathway Form, which is submitted through My Aged Care. Luxe Care can guide you and your family through every step.

Medicare — What It Covers for Palliative Care

Medicare plays a supporting role in funding palliative cares at home, primarily by subsidising specialist palliative medicine services where a GP referral has been made.
What Medicare covers:

  • Specialist palliative medicine home visits: Medicare subsidises attendances by a recognised palliative medicine specialist at your home under MBS Group A24 (items 3005–3093).

  • Multidisciplinary case conferences: Medicare-subsidised coordinated care conferences involving your GP, palliative specialist, and other providers.

  • GP consultations: Standard GP consultation items (including the Level E item for consultations of 60 minutes or more) may be used for advance care planning discussions and palliative care needs assessments.

 

What Medicare does NOT cover:
Medicare does not fund personal care, domestic assistance, or day-to-day nursing at home. These are provided through the Support at Home End-of-Life Pathway or private funding.

 

Important note: A GP referral is required to access specialist palliative medicine MBS items.
