This Dementia Action Week, we are highlighting the action Luxe Care’s Registered Nurses and qualified carers take to deliver the ultimate home care service to people living with dementia.


Dementia Australia’s 2025 theme is “No one can do it alone”. This is the cornerstone of our mission at Luxe Care. People living with dementia find that friends and family often drop away, not knowing how to interact once they have a diagnosis. This leads to social isolation and loneliness. With approximately 433,300 Australians living with dementia, and 1.7 million people involved in their care, chances are you or someone you know is impacted by dementia.


Here are just some of the ways Luxe Care can assist in caregiving and make life that much easier for you and your loved ones:


Maintaining Routine

Maintaining routine is central to dementia care: predictability lowers anxiety, cues memory, and makes life feel safer. We map your loved one’s natural rhythms - wake/sleep, meals, medication, transport, hobbies, calls - into a simple daily flow with preferred carers, timely meds, and meals prepared the familiar way. Discreet supports, such as hydration prompts, clearing walkways, and stove and door checks with gentle communication, help reduce decision fatigue. We offer transport, companionship and can help build purpose and maintain community ties through cafés, clubs, libraries, or churches. Primary caregivers or contacts stay informed with clear notes after each shift and gain meaningful rest with respite.


Personal Care

Our carers provide gentle support for tasks such as showering, grooming, and dressing, protecting dignity while encouraging independence. We work to your routine, helping with bathing, oral care, shaving, hair care, skin care, continence, safe transfers and mobility, and selecting weather-appropriate clothing. With a “do with, not for” approach, we use simple prompts and adaptive equipment to keep the person involved, reduce fall risk, and support skin integrity and comfort. You can request consistent carers, same-gender support, and specific privacy preferences. Any noticeable changes, such as weight loss, new pain, or skin concerns, are documented in shift notes. Visits can range from two hours to overnight sleepovers. The outcome is steadier days, safer care, and the reassurance that essentials are handled exactly the way you would do them.


Nurse- Led Care and Clinical Support

All Luxe Care care managers are Registered Nurses or have extensive nursing experience and qualifications. They provide specialised guidance and clinical care for people living with dementia, beginning with a complimentary consultation and a thorough assessment of cognition, behaviour, health risks, and home safety. A personalised care plan balances wellbeing with independence.


Our nurses can manage medications (distribution, monitoring, reviews), skin and wound care, continence checks, nutrition and hydration checks, swallowing risks and meal modifications, pain and symptom control, and post-hospital support. For changes in behaviour or mood—including agitation, anxiety, or sundowning—we use gentle strategies as directed by primary caregivers, such as validation, de-escalation, sensory cues, and structured routines. With your consent, we liaise directly with your GP, geriatrician, memory clinic, and allied health professionals to track early warning signs of infection, delirium, or decline, and escalate promptly to prevent avoidable hospital visits.

 

Transport to Appointments, Social Engagements or Grocery Shopping

Luxe Care’s Registered Nurses and qualified carers can transport clients living with dementia to medical appointments, social outings, haircuts, or the weekly shop. Our carers provide personalised assistance with preparing to leave the house, including mobility aids, lists, and locking up.


They also help with using familiar routes and building in extra time,so no one feels rushed. We help with follow-ups, scripts, and ensuring payments are made. For social engagements, we can coordinate with family and venues, steer toward quieter times to reduce overwhelm, and accompany them so they can enjoy their outing with confidence.


Hazard Awareness and Problem Solving

Our practical approach to safety protects dignity while reducing risk. Our nurse-led home safety assessment will inform you of potential home hazards:

  • Fall Risks

  • Kitchen/Bathroom risks

  • Medication storage/visibility

  • Electricity/Lighting

  • Door/Lock safety

  • Clutter/Hoarding

We create a simple, personalised plan personalised to the client’s routine. Our team assists with ordering helpful equipment:

  • Non-slip mats

  • Contrast cues

  • Discreet tech aids (stove timers, door alerts, alert mats)


Shift notes submitted at the end of each shift ensure you are always informed, and your care manager will liaise with you about possible changes or solutions if needed (e.g., adjusting tasks to the person’s best time of day, simplifying choices, adjusting lighting, or modifying a walking route).


Care Plans are reviewed regularly, and after any incident, to ensure safeguards remain effective without being intrusive. You and your loved ones can designate preferred carers who understand the person’s cues and calm them early. The result is fewer near-misses, smoother days, and confidence that safety is being handled thoughtfully while life stays as familiar as possible.


Sleepover Shifts

Nights can be when confusion, wandering, bathroom trips, pain, or anxiety spike, so our sleepover care brings a calm, familiar presence from evening through morning. Our carers can help your loved one settle into their usual bedtime routine, maintaining a quiet and safe home environment with soft lighting and clear walkways. They will also step in promptly to assist with toileting, hydration, reassurance, or medication as needed. Your carer rests on-site but remains available for occasional assistance.


If needs are frequent, we can switch to an active overnight service to provide continuous, awake monitoring. We use gentle orientation cues and de-escalation strategies for sundowning, watch for early signs of infection or delirium, and protect skin integrity with unhurried continence and repositioning care. We can coordinate with you to fine-tune night plans and equipment (bedside commode, sensor mats, door alerts) while preserving dignity and privacy. Mornings can begin smoothly—medications, wash, dress, a simple breakfast—so the day starts steady for everyone, and you can get meaningful rest knowing someone you trust is there.


Caregiver Respite

Stress can be incredibly damaging to the mind, body and soul. Many of us feel we need to be able to “do it all”; however, this is not possible, at least not in a healthy way. For families supporting a loved one living with dementia, the demands of caregiving can be exhausting.
