Client Care Perspective

Client Care Plan was adapted for a South Yarra client who wished to spend summer with their family at their beach house in Sorrento. Modifications to care services, including conducting a home visit and assessment in Sorrento by the client's Registered Nurse Care Manager, who then coordinated all in-home care services,

GP and Pathology visits, and service liaison, setting up accompanied travel with a registered nurse to a private hospital in Melbourne for same-day treatment, then transferring back to Sorrento so the client could continue spending quality time with family, using qualified carers and Registered Nurses as support.
