4. You receive a notice of decision with your support plan.
What’s in your support plan?

• Reason for referral
• Summary of goals
• Your strengths
• Assessed needs
The notice of decision includes:
• Summary of aged care needs and goals
• Ongoing quarterly budget based on assessed classification
