Australia’s aged care system is undergoing a significant shift. Starting 1 November 2025, Home Care Packages (HCP) will be merged into a new, streamlined system named Support at Home. This reform is designed to simplify access, improve transparency, and deliver more flexible, needs-based care.

What is the Home Care Package Transition in 2025?

The transition refers to the replacement of HCP with the unified Support at Home model. This change aims to reduce fragmentation and improve the user experience by providing a single, cohesive pathway to aged care.

Why is the system changing?

  • Simplification: Instead of navigating multiple programs, users will deal with one system.

  • Flexibility: Services can be more easily personalised to support individual goals.

  • Improved standards: New compliance measures and provider registration will enhance care quality.

What is the New “Support at Home” Program?

Support at Home is a holistic, integrated home care model launching in November 2025.It will cover daily support, clinical care, and access to allied health services, all managed under a unified funding framework.

Key features:

  • Integrated funding that moves with the consumer

  • Tailored services matched to each person’s needs

  • Robust oversight with clearer quality measures and provider accountability

  • Registered providers, ensuring compliance and service excellence

How Will This Affect Current Home Care Recipients?

If you're already receiving a HCP, your services will automatically convert to the new Support at Home program. If you already had a Home Care Package on the 12th September 2024 or had been approved and were waiting for allocation of a package as at the same date the 12th September 2024 you will remain at your current status until you upgrade to a new level. If you do not meet either of the above criteria then you will transition to a new service agreement under the updated model.

What to expect during the transition:

  • Assessment updates to align with the new criteria

  • Refreshed service agreements from your chosen provider

  • Opportunity for enhanced care, particularly through private-top-up services for those who want it

Case Study: The Benefits of a Home Care Package

Meet the Campbells—a retired couple from Melbourne’s Eastern suburbs:

  • Challenges: Mrs. Campbell struggled with transport and meal prep; Mr. Campbell needed occasional nursing oversight due to chronic arthritis.

  • Under HCP: They received a Level 3 package with weekly domestic assistance, personal care, and allied health visits.

  • Post-transition to Support at Home:

    • One streamlined plan tailored to both of their needs

    • Additional services like garden maintenance and transport consolidated under one provider

    • Mrs. Campbell added private mobility services, funded privately, to maintain monthly visits to family interstate

Outcome:The Campbells now enjoy seamless, comprehensive care that’s more personalised and easier to manage — without juggling multiple service agreements or providers.
